I’ve been working towards going paperless this last year including getting my forms online. The one I still hadn’t sorted out was how to get the contract done – until now I’ve emailed a copy to the parents for review, they’ve printed it and we’ve both signed it, then give it to me and I scan it and email them back a digital copy. But that’s still a bit of hassle, so I had my tech guy working on a solution. (Tech guy is my incredibly loving, creative, patient and talented husband Kit – without him I wouldn’t be able to do this work!)
Well, Kit’s found my perfect solution – JotNot Signature+ has everything I want in a PDF signing App. I create my document, export it as a PDF and dump it in my Dropbox folder. On my phone or iPad I open Dropbox and it has an option to open the document with Signature. Click and there is is! You can add the date, a checkmark, have clients sign (with their finger or a stylus) and save your own signature. Text can be moved, resized, and deleted. Then you can email the signed PDF to the clients, and save your own digital copy directly to Dropbox – all from Signature!
Hooray! If you try it let me know what you think. The link above says it’s $4.99 but Kit purchased it this weekend for $.99 so I’m not sure the actual price.
Update: I tried it out with some new clients and I LOVE it! Quick & easy. The only quirk is figuring out the controls – the signature is automatically dumped in the center of the document so you have to move it to the right line, and resizing text was odd but now it all makes sense. I’m thrilled!

Jenny, I researched doula forms online and compiled all the questions I thought would be relevant from the various doulas who shared their paperwork. I’ve revised my intake sheet numerous times, it’s a constant work in progress as I decide how to better phrase things or figure out more questions that I need to cover. I also get client feedback and that’s helped me refine them.
My husband (tech support!) is the one who suggested putting them online so clients could submit them electronically. It’s made things much simpler to have the forms to review before our first prenatal visit, and it’s helped cut down on the filing of paper copies when information is sensitive. This way I can keep just the digital copies password protected on my computer. As far as actually getting them set up digitally, that was my husband – he built the web form, which involved some server work and programming I don’t fully understand.
